All treatments include assessment, disrobing and redressing, treatment and home-care.

Please make sure you fill out your intake form including insurance information prior to your appointment to ensure you get the most of your treatment time. All paperwork will be sent digitally to the email you put on file.

Please be aware we are not able to determine your coverage or any requirements prior to your visits. For example, some companies and policies require a doctors note, which we must have on file. We recommend contacting your insurance company, prior to your visit, to ensure coverage and direct billing.
